Do I provide the testing and assessment that you need?
I provide the following:
Psychological evaluations to adults who want an in-depth assessment for diagnostic clarity and to provide relevant treatment recommendations.
Psychological evaluation for prospective adoptive parents as part of the required documentation for adoption.
Comprehensive evaluation of Adult ADHD.
I do not provide evaluations custody cases, disability determinations, or immigration proceedings. I do not provide psychoeducational testing or neuropsychological testing. Please see my Mental Health Partners page to link to other providers.
Scheduling your initial appointment. Please contact me by phone at (281) 524-3894 for a brief consultation about your therapy or testing needs. You may also email me at dr.riveraperez@toolboxpsychological.com, but please be mindful that email communications may not be 100% secure.
The initial sessions before therapy. The initial sessions are designed to help me understand your needs and your personal goals. The session is an unrushed face-to-face interview and it may include completing short symptom inventories that will help me understand the severity of your concerns. At the end of this process we will discuss my impressions and recommendations.
Location. 16225 Park Ten Place, Suite 500 Houston, TX 77084. Teletherapy services are offered after the initial session.
Using insurance: To help you use your Out-of-Network benefits I offer (1) “courtesy billing” which means that after your deductible is met your rates are adjusted to the balance of what the insurance does not pay or (2) provide the required documentation (“superbill”) for you to request reimbursement directly.
Payment policy. Clients are responsible for payment at the time of service. Forms of payment include cash, check, Visa or Mastercard. If services are provided via teletherapy, a credit card is required to be on file. Remember that psychotherapy is a reimbursable expense under many Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA), please check with your plan.

Cancellation policy. If you must cancel an appointment 24 business hours is required. Appointments not canceled will be charged at the regular fee.
